DKA Membership Registration

Complete this form after the trial class to apply for membership. The academy will verify the details before creating or linking the Family and Student records.

Parent / Guardian Details

Your verified application has been sent to Das Karate Academy for review. You will receive an email after the Family and Student records are approved and linked.
